That my friend is the real question. All we know is that Yana said the name of O!Ciel is related to the sky too.
Also in some extra of the manga, Yana made a note of some of the possible names she thought before choosing Ciel, so maybe one of them is the real name of O!Ciel but idk.
And the twist would be Yana decided to kill the REAL Ciel and left us hanging for the rest of the manga not knowing the REAL Ciel's name :)

I think its because his blood is a "Sirius" type, therefore they called him "Sirius" And if "Sirius" is his name I think in a flashback chapter it'll be mention but it isn't.
