We can based on the dialogue and inner monologue say he does have a type of body dysmorphia ( his current body and image doesn’t align with desired appearance). That is not to say that he is a man wanting to be a women, but that his physical aesthetic does not match his gender expression whether this is as a cisman male, trans female, or gender fluid or even further categorizes like cross dresser, fem, androgynous. I only say this because of his clear concern in appearing more masculine to circumvent any possibility of that drawing attention to his sexuality whether or not that is definitively gay. Overall he wished for a more slim i’m guessing less obvious masculine physical build to match his young dreams of being a “princess” not necessarily as a woman but puberty hit and left him uncomfortable in himself which can happen for any child, but again that’s psychoanalizing a fictional character a tad overboard. He might simply just like more feminine oriented things and that’s simply that too. But let’s not jump to conclusion about his gender identity just yet.
