I dont think it's unrealistic if you continue reading. He will stop becoming a doctor for a while (or quit ~ maybe) and take care of Zhishu full time, if I'm a doctor, I will do the same for the one I love the most.
money is not a big issue for him, since his father is rich. so maybe he doesn't really care if he cross the line for his job.
The doctor has certainly won my heart, regardless of how unrealistic and unethical it actually is.
The doctor himself is quite sure that he's fighting a losing battle falling in love with someone with a cancer and low success rate.
And well, who wouldn't fantasize about a hot and gentle doctor taking care of them during their weak moments.
Lastly, anything that pisses off and hurts the cheater bastard is a good thing on my opinion.
