It is implied mostly like that but it's not exactly like that. I think Towada has always been subjected to his beauty a lot instead of what he is as a person. So he has developed this sense that he will only be loved as long as he is beautiful. Once he loses his beauty, his worth will reduced. The last chapter is more about him coming to realise that Tani really loves him for him and his beauty is a part of him that Tani loves apart from his all other qualities good or bad. I personally think the hospital scene was to give it more depth to the questions Towada had in mind- doing things just now or it will be never.
It's along the lines of loneliness that binds them together. But it's just my thoughts.
I reeeealy liked the story, BUT!!... Am I the only who thought...
...Towada has some serious illness?? First, there was that hint-like-thing with the movie, where the main heroine died. Then we learn, Towada's real mother died from the sickness. Of course, Towada could have collapsed or felt sick due to the fatigue. But then he appears in the hospital, comes back from the consultation room, Tani claims he will love Nozomi forever, and Nozomi cries... Yeah, I get the scene, and it was very touching indeed! But at the same time I was pretty sure Nozomi just got some awful diagnosis from the doctor, and it will lead him not to the death, but maybe to the ugliness? Errrr... So I'm a bit confused by the happy ending. I'm happy for both of them, I really am! And I liked the story a lot. But it's just like "wow, was I overthinking it so much?!" kinda feeling X')
