I looked it up and apparently anxiety disorders and/or OCD can cause people to constantly be itchy. Not sure how savvy Horikoshi is to mental health stuff, but AFO said that the itch was a physical manifestation of Shigaraki's urge to express his quirk.
If I remember correctly he was really only itchy at home, and once his father was gone he felt relief and didn’t itch anymore, so it was probably linked to anxiety due to his abusive father that manifested as scratching. But that’s just a theory.

A close friend of mine has itchy skin and she has been through a lot of shit in her life... In her case it is a severe case of anxiety and ptsd, wouldn't be surprised if it was the case for shigaraki too
Yes bitch I do Stan some villain character development. Barely understand what was happening in this chapter but I do understand that someone needs to get crusty lips some chapstick or something. Also just a little thought/theory what if over the time crusty lips uses his quirk he himself starts slowly decaying and that’s why his lips can’t be saved and he’s always going itchy itchy
