It depends on the Artist. Some mangakas draw them with balls, some dont. But most do for anatomy sakes, even in omegaverse literature the issue of the scotum varies from its not producing viable sperm to it being completely atrophied due to evolutionary changes with the Omegas.

As far as I know this is the omegaverse "dick rule": Alphas have the length, the knot, and under/after the knot is the balls. the knot is like the lock-like thing when the omega and the alpha have sexual intercourse/bond so it would be sure for the omega to be pregnant. Betas have the regular dicks while Omegas have the small and straight (like small-to almost no balls) dicks. First of all, omegaverse isn't a single strict thing, the universe of ABO depends of the artist. In this universe, they do have balls; Just because you read another omegaverse that the omegas doesn't have balls, that rule don't necessarily applies for ALL omegaverses out there. The rules change from manga to manga. Another example is that in this universe, betas feel attracted by the omega's smell too, but I read several other omegaverses that the betas are neutral about it. So what, the mangaka is wrong? Nope! And the dick redrawers don't, too! As I said, there's no such thing as unflexibles rules in that. Stop bitching about it, accept the facts, lol ┑(￣Д ￣)┍

If the diagram were in this manga, explaining how the artist choose to be his omegaverse, you yould be right, but otherwise, again, your point doesn't applies hete.
Yes, diagrams are there to be a rule, but to show how the specific mangakas built them. There's the ones where the knots exists, other that It doesn't, some where the fated pairs isn't a thing. And none of them até wrong, that's what omegaverse is about.

Oooh! Actually, I think I know what diagram you are referencing. It's a super common one. It shows three different versions of genitals all with penises and one of those does indeed lack testicles. But the one that lacks testicles is always labeled alpha/beta/omega women with an explanation above it that says the women in these worlds have adapted to have enlarged clitorises that function as penises. I don't remember the details of the explanation, just that I found it interesting that the women have penises. When I first noticed it I even went and cross referenced it with some other manga that have the same diagram to make sure it wasn't a mistranslation. Maybe that's what you're thinking of?
