Following option A, they might not really be the "last" omega and alpha though?
The alpha is the director of the lab who produce the medicine and *spoiler* kinda, sister works there (she is who gives it to the omega)
Sorry, this might be a dumb question, but if omegas and alphas were already "extinct", why is the production of suppressants still ongoing?
Possibilities:
A. There is an existing demand for these medicines with very few existing omegas and alphas hiding their genders from the society
B. The betas found other uses of the suppressants, thus the continued production
