I don't think that's the real doctor. It's probably an impersonating shape-shifting vampire. Remember the real doctor is supposed to come the next day? Also, it seemed like the doctor was trying to trick Doyoon into telling him how old he was when the doctor last treated Doyoon, which is information the real doctor would have already known.
