It probably had some opium or as they’d call it in the olden days ‘milk of the poppy’. Arina was already taking medicine, so it wouldn’t make sense if it was just medicinal. Especially since the duchess felt guilty giving it to Arina. If you’re looking at it broadly the candy was also medicinal because it was used to numb her pain. You know how Arina got better and then died? That’s because she wasn’t in pain, she was going to die either way but she got better because she wasn’t in pain because of the opium candy.
