I think that it’s a matter of how common is it to have a slave that can read and write. You’re correct, however, what are the odds is he going to meet a slave who used to be Nobel vs a slave that was born into slavery.

I don't even think its about her being a noble. Jorge believed the lie she told him that she used to work for a doctor before.

I don't remember either. But Jorge didn't question too deeply why she could read. She just lied about working for a doctor a before being a slave. She will just tell the Archduke the same thing she told Jorge.

So from what we saw in the first chapter they have been making all sorts of people (that they don't kill) slaves, even former noble women. Why is he surprised that she can write and read? It is not like a person has to be born a slave to become a slave in the empire. He doesn't even have to assume she was once a noble. It is simply that before she was made a slave she was taught how to make medicine.
