Yes he understands everything that is happening the only problem is that they have a language barrier. But at the end when he eats a piece of Aka they are finally able to communicate verbally and you realize Aka has always loved him in that sense.

Sorta like he has to consume a part of Aka. And that’s when he gets his youth back and become basically immortal
Did Aka understand the meaning of what they are doing together ? I know it's fiction but at the end of ch2 and ch3 when you see Sakichi do that, did aka understand the thing or he was just going "with the flow" (which kinda weird because he is not fully developped yet (his mind))...
