Who is he? I mean I know he is the doctor, but why is he hated so much?

Some hate him, others just don't like him. His role in the novel is not a good one, he encourages something that shouldn't be encouraged and when he saw the Duke (later chapters) happy and nicer (because of Lucia) he was not happy. In short he represents the dark side of the Duke family

later in the novel you come to find out he’s super obsessed with continuing hughs family line. At one point he even brings girls he thinks hugh might take interest in, gives them the needed medicine to get them pregnant. but since he can’t impregnate them he immediately kills the girls after. pretty shady and crazy guy

Lucia gave birth to a baby girl later. I think what she meant is that Hugh is only interested in his wife, so the doctor probably got rid of the girls later on to avoid suspicion.

When Lucia was young and had her first period no one was looking after her so she didn't know that it was normal and took some medicine to stop it, then Philip(the old doc) convinced Lucia personal doctor to give her an antidote. Therefore she can now have children. Hope this clears up any confusion.
