i mean he has poor health like his mom i mean that's what they said in the latest chapters that's why he wasn't able to go out and be as active as his twin brother cause he had some kind of illness and his body wasn't strong and i remember in the atlantic arc where they were in the ship ( i think ) Ciel coughed and sebastian was worried and he said something and ciel told him to shut it cause lizzy was near them it was probably not to point out that he was somehow still sick and lizzy would notice cause before the whole murder of his family everyone knew that the fake ciel was sick
