He probably doesn't recognize her because she can use a change-face skill. She probably used it all the time after returning to the palace, maybe to look more masculine since she's supposed to be a man and her face clearly isn't one of a man.

honestly. I've read the spoilers and it just seems slightly messy but I think it's kind of needed? it just adds more depth to the story. at least for me. yes I do get why some are dropping. some are also saying that the male and female version looks the same how can the emperor not know. it's fiction. the author could've drawn her that way so it's easier to show for us but we have to remember. this is a manhwa. anything kinda goes. . I love this manhwa. it's honestly very beautiful to me. :)
