So basically in their past lives they met when they were young and met again around their early 20s. They made love obviously. However sungyeon was a noble, and had a wife. After that Sungyeon and his wife had a child. In this life Sungyeon prioritized his responsibility as a noble than his happiness with Hosik. So past Hosik was the one who took care of his/their child. Until the kid was able to walk and talk. From what I had observed he was ill. Idk maybe it was depression or something, but he had something going on and his health deteriorated. When past Sungyeon left, Hosik went to see him off. And by the time past Sungyeon returned, past Hosik was already minutes away from dying. Past Hosik and Past sungyeon talked for a bit , but Hosiks last words were "I love you" and died. He told his wife to close the door, and his son saw the reaction of past Sungyoen when Hosik died. Sungyeon took of his headpiece and kissed past Hosik dead lips one last time while crying. Past Sungyeon cried, he must've regretted it....
