Actually karino didnt have sex with that guy. He tried to help azusa, but to get the key he must take it from that guys hole. And thay guy called eno make a call to azusa while pretending that karino did him while he just searching the key. And its too late after all.
He had to get the key to save him, he had no choice, and he didn't actually have sex with him, the other guy just made it seem like it to make Azusa think they did

This is actually the first chapter I've felt genuine emotion about. Azusa didn't deserve that and on top of that, the person he likes ditched him to have sex with another boy. Karino is trash and he better do a whole lot to make us (me AND Azusa) forgive him
