
It's hard to say since every person who had DID experiences it differently and because of that, there isn't an idealized image of it?It's hard to define what an "extreme case" would be, but I find this story to be somewhat realistic since it portrays alters having different thoughts, memories, and actions as well as ages and genders.

There are few chapters BUT right now it is pretty acurate this is how DID looks.
I would like to add since the manga hasnt brought it up yet that DID is a coping mecanism that the brain creates for children that were abused so they can dissociate from the experience. It is not something u simply get and overall victims arent agressive.
Like it was refered depending on the stage on which the system is, being treated like they are not normal or human can have extreme reactions.
Usually if someone knows that they have DID they are able to setup a system of cooperation so they can keep a schedule and not run around doing whutever they wanna do. But everycase is its own thing
Is this an accurate portrayal of DID? Or is the author just using an idealized image of DID to make this story? Is this an extreme case?
