Ok, so I think it has been said enough times that Hugh has a son, who is actually his nephew.
His twin brother Hugo had a son with who was presumably their half sister or cousin (or someone related to them by blood) .
Apparently Hugo hadn't known his Lady was related to him. It's possible they (all the adults at the time) hid this fact from the brothers and the girl so that they'll continue the bloodline.
Which is probably one of the reasons why current Duke Taran (Hugh) wants to end the bloodline. It's messed up and not sustainable.
The doctor, on the other hand, is obsessed with keeping the bloodline because he's somewhat distantly part of the bloodline (human descendants), and because that's what his family has been doing for generations. That, and because the Tarans went through a lot to rebuild.
Personally, though, it's not as if the bloodline would end even if Damien's bride isn't related to him by blood. It is mentioned that a Noble and human would get daughters, so it's a matter of changing the right of succession so that daughters can inherit the title and continue the dukedom. If the doctor means the bloodline as in the power they retain from being once magical, then yeah, that would be lost over time.
The inhuman strength of the bloodline would probably dwindle as a result, but at least you don't have to force all your descendants to be incestuous.
