I am not a BnHA scholar, but I think I can answer your question. I get what you're saying because in the anime they were all, "though, the Hero Killer is gone, his ideology continues on," and then it panned to Toga and Dabi and stuff because he inspired them. But I don't think that they meant he died. They just meant that he is gone and his message is still here.
And writing this out, I understand why you thought the Hero Killer died but I think that he is just being put on the back because he will come back later. This is just speculation but I think there is going to be a Prison Break Arc when all of the villains who were locked up are given another chance to cause havoc.

He hadn't died, he had actually passed out. The police captured him and put him in prison so he was never mentioned again
Actually he has been mentioned a lot of time...
He is imprisoned. When All Might defeated One For All and got to see him in the jail we even can see Stain in one of the panels.

Why is the Hero killer still alive?
First off, I got through the anime first and then read all chapters that happened after season 4. Now I’m starting from the beginning of the manga and I’m at ch94, lol I know totally backwards but whatever.
Anyways, I’m currently on ch94 pg17 and they just showed the hero killer???
Didn’t they say he died in the anime? In both the anime and the manga it was said that a rib pierced his lung but there he is????
Am i missing something?
