The "Assistant" guy (Long-braid) is a traitor of Cain.
He has a different objective and goal.
He carelessly gave away the compass (that he got from Cain)
to someone unreliable (so unreliable, that the one the Assistant gave it to was the one who chased after Police MC and Cain somewhere in Chapter 5-15 or 20 I'm not sure.)
That compass was something very powerful, and Cain entrusted it to Assistant to kill the Doctor,
But instead, the Assistant just gave it to the unreliable guy (the previous enemy) to do the job instead.
The reason why the Assistant really wants to kill the Doctor right there is because he's scared that he will mess up his plans because he might know something that Cain doesn't know.
So, the Doctor is trying his best to gain Cain's trust and to successfully stop the Assistant. That's why he was trying to outsmart the Assistant.

Cain wanted the doctor found to cure him. The assistant, Sid wants the doctor dead to take Cains place.
