I was thinking the same. Maybe he was adopted similarly to her? Or he was illegitimate? Or he was simply a runt-like son-of-a-duke who liked to meet and play with the peasants? But yes, I'm guessing the childhood friend too, until proven wrong.

i dont know because i was thinking that he was the childhood friend but when he said she seems familiar he had a vision/flashback of a grown MC so I'm a bit skeptical

Well, there's also the possibility that he met the real Karina before her death, a random encounter. Not enough to truly remember her by herself but enough that she looks met-before, just one of the many people he crosses path every day.
