IKR!! It happened when he first met Pauliana and she was just an enemy prisoner to him. At the time he didn't care for her and ordered that as a part of her execution (harsh i know). Of course she fought back hard and managed to survive, then he made her a knight (as we saw in the manhwa).
On the bright side, later in the novel when they become close he expresses that he regrets ordering that when he first met her.

I just can’t take it with the ending. I rarely drop shit LIKE VERY RARELY EVEN SHITY YAOIS. I love all of them a lot especially Paulina (pls be my wife) but I can not take it anymore. It is annoying and painful to know what will happen. I believe if he truly loved her he should’ve told her and not just do what he will do. I’ve said it before but I really hate the concept of having multiple concubines and it is just a personal opinion. It breaks my heart. Sorry.
