Ci Nian does have ovaries since he can give birth. If you consider the difference between a man's reproductive system and a woman's, Ci Nian probably doesn't have testicles but does have a penis. And his testicles were replaced with ovaries. The difference between a man's and a woman's is basically a man's is just outside the body. Hence why a man can become a woman and a woman can become a man. The vagina is just pulled out and the ovaries and uterus probably removed and a man's is push inside and balls probably removed as well. Another reason could be because similar to omegaverse, male omegas can become pregnant through their anus so I guess the best way is to consider his body alone as a male omega but without a heat cycle and pheromes.
