Same thoughts. It's super strict. I watched Umibe no Etranger - The Movie last week and Japan rated it for 12+. The sex scene was included.
I got disappointed with the 16+ rating for this.

It was in a super-fast pace imo. Coz they had to ram the whole volume in 59 mins. But it was satisfying. Every frame was picture perfect. I didn't expect the backgrounds to be sooo detailed. Like paintings. It gave me a Ghibli x Makoto Shinkai feels. Of course no pp were shown but the whole scene was there. No sugar-coating at that. "Seksushiyo" (Let's have sex). Lol and thr part about condom and ass-prep were there. I couldn't believe that it was R12. Then again, it's some kind of raising awareness too. So it's nice.

Oh no, sad to hear that. I hope everything goes well on your end as soon as possible. The cinemas here are always half empty. One-seat-apart and staggered. So there's no one on your left, right, front, and back. Still a bit scary but with proper precautions and sanitation. I think it's okay. The theaters have automated temperature scanners. So yeah. Kind of awkward when you have someone to watch with tho. But we have no choice.
