yep, Siyeon/Penelope decided to stay in the game world. She didn't want to go back to the real world/Korea. This was due to the abuse she suffered under them, and she knew that her real body only had a few days left to live. Therefore, why go back when she will die anyway in the real world?
Also, she wanted to stay in the other world because she had all the love she needed.

i read a spoiler thread about this a long time ago and one of the things that made me rlly sad is that our female lead in her real life develops stomach cancer and her family hire the best doctors and they stay by her side and at the end of the story she gets a choice of whether she would like to go back to Korea or stay she chooses to stay since she decides to stay i think she dies in her real life
