That doesnt explain why the doctor was there tho, I think he tried to kill himself
Actually maybe his top knot was cut and that's when he tried to kill himself so they were holding him down and called the doctor

Just had a thought, about the flashback moment when SH is having a rage/breakdown, and is being held by the servants. The knife next to him. What if this is right after his father cut off his top knot with that knife? His hair are a mess, and strands look freshly cut to me. He's so pissed. And, perhaps, that was the moment he was referring to, when he said he should've killed everyone and himself?
