I think it's more than the issue of homosexuality. I think he was raped by a man when he was a child, and it was his father that put him up to it. That's why because of trauma, he became the person he is now. Remember from the old chapters when his brother came and then he said something along the lines of "I'm living exactly as the way father made me to"? I think that was a premonition about his past, about how his father offered him to the person that Jihwa(?) imitated in his note/letter.
If we look at it in this angle, we can explain how he got worked up becayse of one letter from an old dude, how that Jihwa said he accepts him despite his past, and how he said he hates being touched by older men.
well technically homosexuality is seen as taboo in korea, up until these days. kinda funny when you think about how much yaoi manhwa korea publishes lol. but in the old days (joseon era, setting of this story) it is wrong to be attracted to men and some see it as "illness". i dont think he's gay in the first place, he fucked men because he resented his father, not him attracted to men in the first place.

Looks like he comes from a very prestigious family so probably something like homosexuality is unacceptable and something very shameful... Could it be that they tried to ,,cure" his homosexuality (ridiculous as fuck) so they took him to the doctor but the doctor couldn't do anything so they locked him up in the house and gave him drugs which they though would make him ,,normal"... It's scary how narrow-minded some people are.
