So I've skimmed through the Raws and from what I could tell, that injury symbolizes a lot in the story.
It's like it's hidden under all that bandage and that's why at first the seme really acknowledge the feelings of the uke.
It's like the uke has been hiding that nasty injury under a lot of bandage and has not taken proper care of it, instead he numbs the pain with alcohol.
Also, I've noticed that the seme often touches the uke's injury, I think it symbolizes how he always pulls the uke back into that harsh problem everytime the uke tries to heal. It's like he pulls him back in it and makes it worse for him everytime.
I love how much meaning the author has added into that detail, it's crazy how the seme only ever reaches for that one hand delibaretly to catch the uke anytime he's about to get away from him.
