Shes mtf meaning she transitioned into a female. That's why the guy has a hard time calling her sister and not brother. The surgery and hormones were to make her more feminine

At first I assumed the sister was a transgender male and that’s why she left the house, (also because She has big boobs, and I didn’t think she had surgery.) but I’m reading the comments and apparently a trans female? Someone please clear this up for me!!
