Its so ugly it physically hurts
I think it is just badly drawn and place in a wrong body part, if it is like that of Kakami (Kuroneko series) it is fucking hot.
At first I thought it was scab and I was thinking that he got it when he was injured.
but like seriously, who thought that a cheetah print is a good tattoo design...
