I don't think he choosing who to give the medicine to. He's just trying to decide whether to give the medicine to the old man (because he has a limited supply and he doesnt want it to leak to the public), and whether or not he'll go to Japan (since he's worried about his sister's mental health).
Apparently the translation of the last panel was just a lil weird, according to some other people who read the light novel.

If i remember correctly he has 6 of those medicine he used to cure his mother am i right?? Well correct me of im wrong cause if theres six more of those then theres no point in jin woo choosing between the two
