There are josei/smut out there that don't focus on size. They may not be popular, but it's there. As to why bigger seems better in fiction, it's honestly just the way it is. Obviously, the mangaka's preferences can factor in, as well as what's generally popular with the public. Speaking on a personal level, average is best.

I just wanted to say, why must every ml in a josei/smut have a big dick?? For fuck's sake I'm not saying a small one woud be good (by holy I don't have any prejudice about beign big or small) but in this kind of mangas, even if being big would be preferable, it makes one belive every guy has it big, and the small one's are just a myth
