In the manga it explained that Hikari is "intersex" meaning he has both male and female reproductive organs. For example, he can have a penis but instead of testicles, he would have ovaries. In the manga they use the term "hermaphrodite" which some still use while others say it's a derogatory term. I hope this was helpful!
