Clearly he was the one who left her when they were younger so why would he mention *her* leaving unless he was talking about their previous life? I mean yes she did break their promise but I highly doubt there is any way that could be taken as 'leaving him'. Plus both she and Lancelot use the words 'second time' a lot so it's more likely the start of the first chapter was showing her current life, clearly.
