Like with what has happened to some insults worldwide, the term Okama has normalised and has acquired a neutral sense. It is a matter of context and intention when used. The term itself is quite vague and is relatively slurrish but it basically means gay, trans and cross-dresser.

There is a term that is commonly used in the media and by the queer community to refer to drag queens and gay/trans people, it's '' onē '' pronounced 'oh-neh'. It can be used as a derogatory term but many in the queer community use it as a lighthearted referential-term. However if you just want the term that is widely recognised in Japan to refer to trans people, then the term is '' TORANSUJENDĀ ''. However I don't think that Shinobu is trans? It's not proven exactly what they are.
The misgendering thing is also not a serious issue (refer to chap 1, pg 6)

For the fight situation, I believe that she wasn't trying to expose the person but that is the only term to refer to gays, cross-dressers or trans people. Since she doesn't know which of these Shinobu falls under. Mc also used the slang okama and 'onee' which refers to effeminate men and she also had a similar prejudice towards these people because of the representation of them or the stereotype around them. (chap 1 pg 7)
our mc is also not especially close with Shinobu as shown on chapter 7 pg 10-12, she also uses the honorific 'san' which is commonly used when speaking to or refer to someone who you do not know very well but respect (like a customer, boss or colleague) so the friends worry and argument is reasonable.

And for the Sherlock scene, take into account that this is a manga, so it's obviously exaggerated to show how overprotective the best friend is (this fact of being overprotective can also be seen on vol 2 chap 8.5). The friend has many reasons to be worried, one being that she thought Shinobu approached our mc for evil reasons, another being that they are a adult 'man' hanging out with a minor (tho i doubt the 'minor' part is an issue) there are many things to take into consideration.

However I feel that the main reason for all of that is because she is scared of losing her best friend or 'child' of 7-8 years to someone that mc has just met. She is scared of the change that her best friend is going through and most likely believes that mc will move on or grow up without her.

I don’t think she’s trying to “expose their deadname” she specifically said that they might be a man pretending to be a friendly okama to get closer to girls, to me she seems like she’s looking out for her friend.also japan is a pretty close-minded country so it’s not unusual that they have this kind of stereotype
