Ouh I forgot to add, mc has to overcome his trauma of scalpel bcs he doesn't want ml to die fr. Maybe some obstacles happened. Ml went into coma. Mc blame himself. Bro depressed. But after a while ml woke up. They fuck all night long and ml decided not to do gangster shit and live a normal life mc... He may not continue performing surgery and just the usual internal (....I forgot the name) ...I have so many ideas.

First of all, The Marriage of Figaro is an opera. No one is getting married. Jean and Theresa are singers/performers in the opera. Gepetto called himself an artist and I'm not sure what role Agnese has.
Starting from the beginning:
The director is mad at Jean for changing the lyrics of his song in the middle of the performance. He yells at Gepetto to get Jean. Agnese jumps in and takes the blame for changing the lyrics. In exchange for helping Gepetto, Agnese asks him to give the handkerchief to Theresa in the green room. Gepetto seems to know she has not so nice motives but does it anyway. He sees Theresa sucking Jean's dick, leaves the handkerchief, and runs off.
Jean knows that Theresa is the one who came up with the plan for Gepetto to see them together. Theresa talks about how he's fickle and she can't believe his words. She needs more proof that he likes her. Cut to the performance where we see Theresa with hickeys on her neck. She's openly staking her claim on Jean to the other singers.
Flashback to when Gepetto first met Jean. Jean is hurt after being punished for stabbing a pedo priest. He asks Gepetto to use a cigar to stop the bleeding. Gepetto is reluctant to do it but eventually caves in. Jean kisses him as thanks. While Gepetto is sleeping, he leaves the handkerchief and runs off.
Back to the present. Agnese is crying because she knows Gepetto is a nice guy and she set him up. (She might even be crying because she thinks Gepetto deserves better than a playboy like Jean.) Here we learn Agnese is Theresa's sister. Gepetto consoles her and says he's used to stuff like this. As the lyrics "accept from me this diamond ring as a token of the love that I pledge to you" come up, we see the scar from the cigar around Jean's ring finger. I see it as a symbol of Gepetto and Jean's unyielding bond. Gepetto also notes that bruises will vanish (Theresa's hickeys) in contrast to the permanent scar he's left on Jean.
