You're not stupid. This is my fourth time trying to write this because Firefox hates me today. The others were longer. To answer your questions: nope and they've never had sex. Whenever Ryouta is mean or sexual those are just hallucinations because Acchan feels so guilty about the accident. When Ryouta's eyes are swirly, which is most of the time, he's basically helpless. We've only seen his eyes darken a few times in the manga. That's probably close to what he would have been like without the accident and amnesia. It's more of a back to your old self kind of thing, not a double personality. So, basically the helpless Ryouta we see is real and the sexy cruel Ryouta is all in Acchan's head. Real Ryouta doesn't even remember what happened. Hopefully this helps a little :). Let me know if anything is unclear and I'll try to help!
