Ideally for me if Shinobu has to end up with Kou, I would say Shinobu and Kou talk at the hospital, Shinobu forgives Kou but refuses to get back together (says goodbye again on good terms). Kou learns to be a good person, stops playing around (like, there's nothing wrong with having flings but going out there to intentionally hurt people and calling them all your "toys" needs to be fixed.) and spends time doing good deeds. Timeskip two years later they meet again and mend the relationship and get together. The rest would be time Kou proves to Shinobu how much he loves him and is now a good person to EVERYONE.
TBH we all know how this is going to end but Shinobu deserves someone better like Kenji. I don't feel sympathy for Kou he played around to INTENTIONALLY hurt and discard Shinobu he doesn't deserve a second chance. I can't see someone who singles out an innocent virgin who lost his first love, stole his heart and trampled and spit on it. Keep having sex with all your sex friends Kou have fun til you get your deserved STDs
