I think you mixed the timeline. If I'm not wrong, the beginning of Dear Benjamin, when Felix meets Isaac in his flowershop, takes place AFTER the events of the Island and thus after Issac's first heat. So it's normal that he already uses heat suppressant pills. no ?

Anyway the main storyline takes place four years after the incidents of the island (when Isaac got his first heat and got pregnant with Felix's child). So he already knows he is an omega and uses heat suppressants.

Uhm a lot of things happened so I will try to remember the memorable parts
-he gets hurt a lot of time like I mean so many times to the point where he is mentally unstable
-he has a stomach issue but Gu doesn’t care
- after destroying the piano there was a birthday party I’m not sure for who but Gu and that other boy Shen play the piano but it is Nian Nian song that Shen stole from him, Nian Nian confronts him in front of everyone and Shen says that Nian Nian is crazy and is lying and Gu believes Shen so after that Nian leaves and goes to the bar and tang Shuo sees him there but after talking with Nian, Nian collapses because his stomach hurts,
- after spending so much time in the hospital he returns home and Gu says that he didn’t care that Nian was missing,
- I’m not sure but Nian NiAn hangs out with Tang and Shen catches them and takes pictures to show Gu which then when gu saw them he was mad and went home to confront Nian Nian while the confrontation he hit abused and yelled at Nian Nian... this is what happened I’ll just put in quotation mark what exactly happened “ Gu Yan suddenly showed a strange smile at the corners of his mouth, grabbing Wen Niannan's hair and dragging him to the basement” after being put in the basement Nian was yelling in Terror because he did not want to be put in the room where gu was going to be put. Someone comes to Nian rescue after the butler called and took Nian to the hospital since Nian had a concussion

- memory is distorted for me right now but I think that in chapter 80 Nian Nian finds out that his wedding ring was a fake one given by Gu

- There is a banquet that both Nian Nian and Gu had to attend to and Gu decided to take Shen to it. During the banquet they didn’t cross each other at all until a group that hates Gu tried to kidnap Shen because they thought it was his wife, after almost kidnapping Shen Nian Nian shows and Shen tells the kidnappers that Nian is the wife of Gu therefore they should take him. they do take him and no one ever notices. WARNING BLOOD Okay so when Nian Nian was kidnapped the kidnappers thought that they would get a lot of money out of Gu to save Niani but they never received anything from him. OKAY BLOOD/ABUSE PART I’ll just add the part , “ He wanted to run to the door regardless of the injury on his body, but was kicked to the ground by Sun Qi” “ Wen Niannan looked at the white stained red with blood again, which was like the pain of those years. He curled up and hugged his head and cried.” “ Tears mixed with the blood on the face slowly slipped down, and the bottom of the eyes was full of unbelievable, and the sense of despair spread to the whole body in an instant” MAN ITS HORRIBLE WHAT WEN GOES THROUGH!! Okay so after a few days Gu starts getting word and gets told that nobody had seen Wen Nian Nian, so many days passed that after awhile Nian Nian comes ..... I’ll paste the things that happened

-Nian Nian sees the necklace on Shen and this is where Nian Nian starts getting mad
Nian Nian says , “ Gu Yanlu... Do you really think I'm so cheap? In the past three years, I have ignored the humiliation and curse again and again. If I hadn't loved you, how could I have humbled for so many years?”
He also says ,”

After saying this Nian Nian says he wants a divorce and Gu goes crazy ... Gu’s mother comes and sees Nian Nian ... nian Nian tells the mother that he want a divorce and the mom said that Nian just needed rest ... Nian puts a knife to his neck and told her he wanted a divorce, the mom agreed and gu signed the papers. Nian takes a shower and leaves the house and this is where Gu sees him in the street and saw NiAn collapse so he took him to the hospital
They go to the hospital and the doctor tells Gu that Wen went through inhumane abuse.
OKAY NOW ITS JUST GONNA BE BULLET POINTS
• in the hospital Nian was mentally distraught so he attacked anyone who got close to him
• Tang found out and stayed with Nian and Gu was not allowed to be close to Nian because Nian was scared
• Nian recovers slowly in the hospital and tells tang that he is W.E (the producer who makes the piano songs that Gu loves )
• still being in the hospital Nian saw that Shen said that he was W.E , Nian was speechless so when they got out of the hospital W.E(Nian) tells the world that Shen is a imposter.
• Nian comes out as w.e and Gu is shocked
