I'm pretty sure the original guy fell into a "coma" kinda basically and the psycho personality took over and still lives on. I hope that makes sense, if you want I can try to reword it for you

Ok so you know how the main character was able to switch bodies with other people and he got stuck in that guy's body because the other guy had 2 personalities? Well when he killed the other guy, the personalities switched last minute and he killed the normal personality and the psycho personality is back in the original body (the body the main character is currently in). Now the main character is forever stuck in that other guys body. But the main characters conscience basically gave up in life because he has no hope since he doesn't want to be forever on the run and the psycho personality that was in the main characters body that is now back to his original body took over and is now currently in full conscience while the main character is basically in an eternal sleep. If that makes sense now

after reading the comments, i finally understand what's happening. so yea, deku realised he has feelings for todoroki because he can see the spirit who disguised as todoroki. one can only see the spirit if it disguised as someone they're in love with. which mean, this also implies to kacchan who can see the spirit; he likes deku too
