He knew that cause Roxana can't stop spitting out blood and as what chapter 21 has shown to detect any impurities, and he knows Roxana's body is way too poisonous and her organs are badly injured because of her consuming way too much poison...
And major spoiler: The FL, Slyvia, Cassis younger sister has this kind of power herself.. Healing/getting impurities through kissing.. But Cassis' ability exceeds the FL.. Cause he is able to heal Roxana and getting impurities faster and better..

The little boy (commoner) she like is not taking advantage of her. It is true that I actually was sus of him in the first chapter since he agreed so quickly. But he revealed his own point of view and thoughts of what happened and I believe it’s true that he actually does love her. His reasoning went along the lines of “I like her, she’s so cute, she likes me this much, I’ll never find someone else who would like me as much as her, and I’ll make her my treasure I want to protect and won’t let go”
