It's been a while since I read the novel so take what I say with a lump of salt. The doctor comes from a family who's goal is to make sure the MLs family line continues. The ML despises his family for how messed up everything in it is, and wants the bloodline to end with him, but the doctor wants him to have kids. Like I said, his bloodline is obsessed with it. They're purpose is to make sure the bloodline continues and remains pure, hence the inbreeding. But the ML can't have kids because no one was prepared for it, I think all the girls they prepared were killed. So the doctor wants to do something shady to Lucia and the ml is aware of his intentions hence prohibited him from being anywhere near Lucia. But now that the female doctor is consulting him, he'll find out Lucia's body is already prepared for having MLs child and will take advantage of the female doctor and try to give her a medicine that will bring her period back and get her pregnant. But Lucia knows what the medicine smells like and refuses to take it at the first attempt. So the doctor creates another medicine that has no scent, and calls it headache medicine so that she consumes it. It works and she later got pregnant and the doctor was thrown in the dungeon and worked in there till he died. The female doctor was fired sometime around the second attempt.
