Well, there is the problem of Kaoru possibly relapsing. He already disassociated from reality due to his intense grief and guilt. Most people with his illness are non violent and are not a threat to society but for their safety, it's best for them to attend therapy.
I have a feeling Akira is already getting therapeutic help. He mentioned a facility. It might be a voluntary in-patient mental health hospital.
