I think it's about his wife/fl's mom. We haven't gotten much info about her yet and chances are high that she died of some illness which broke the Duke's heart.

The reason is that the daughter once saw the duke with her dead mother as a child or something in that vein of thought. The moment she saw the father standing over the mother’s dead body covered in her blood, Juvel passed out. Either she forgets the memories from trauma or he wipes her memories of those events. I think he ignores her so that she doesn’t remember what memories she lost. I think the duke did kill the mother, but he was forced to by the Emperor through mind control. Anyways, he loves her, so seeing her pass out in front of him maybe he thought events were repeating themselves.

the duke was supposed to kill his wife but couldn't bring himself to. as a result the two imperial guards that were in the room did it for him and as a result the duke killed the two guards. juvel happened to see the blood on her father's sword thinking he killed her mother when he never did.
