It was just showing the night before that he wasn't feeling well so he went to get an IV drop (It's common in China to get this done if you're coming down with a cold, or feeling sick, as it's believe it will make you heal faster). Lihuan was so focused on not letting Yuyang know that he wasn't feeling well that he forgot about his birthday. In Yuyang's mind, he doesn't realize Lihuan is sick, so he takes his distance and forgetfulness as a a sign that they were somehow drifting apart in their relationship.
Those chapters are just showing that no relationship is perfect, especially when you've been together for many years. The passion starts to diminish, life can get busy, and this can result in doubt and miscommunication. But in the end it also shows that they both still love and care very deeply for one another.
