This is accurate. Read the novel a while ago and things don't get better for the MC. It was really depressing reading his pov bc he's just so... broken. He even becomes an unreliable narrator bc he really was losing his mind. The hyung gets a grip eventually when he realizes the MC genuinely is mentally ill and starts caring for him like a patient

You are in fact correct. Mc and his sunbae were involved in a building collapse. The sunbae died, mc is still alive but dying, which is why he's having those episodes where his nose bleeds and he vomits blood and passes out. His sunbae is the ml, which is why he falls for him so fast and hard, because he was in love with him in the real world, and possibly already dating him (not sure, haven't reached that part of the novel yet). There's a scene coming up where he has a major episode and hallucinates, mixing what he sees in the real world with what he sees in the novel, and is freaking out that the hotel room they're in has collapsed and that his legs are pinned/crushed and he can't move, that the building is on top of them, and there's blood pouring from ml's head but he doesn't understand why they can't stop the bleeding.

this might not be completely accurate bc it's been a while since those chapters, but basically the mc/beta genuinely liked the alpha, but the alpha lead the mc/beta on and constantly took him out on dates and stuff and then left midway to go see the omega (so he was neglectful, etc) because he's been in love with the omega for his entire life bc he grew up with the idea that an alpha belongs with an omega.
the omega orchestrated this by purposely calling the alpha out when he knew he was on dates with the mc. he used to think of the alpha as a little brother, but hates being stereotyped as an omega, so started hating the alpha when the alpha wanted to become mates. became interested in the mc because the mc didnt just see him as his gender, but the mc simply saw the omega as a hyung.
something something, the alpha repeatedly and nonconsensually knots the beta and basically keeps him in his room without letting him go until the mc has to go to the hospital. this was caused by his rut but also bc the omega drugged the mc at a party and made it so the mc could sense pheromones and couldn't control himself, so the omega played "savior" and "saved" the mc by sleeping with him to get the drug out of his system and left hickeys or whatever that the alpha saw and got mad at. after the whole situation, the mc didn't know about the drugging and the omega used the situation with the alpha to put himself in a better light and date the mc as the "perfect boyfriend who would never intentionally hurt the mc". eventually the mc finds out about the drugging yadda yadda and we end up here.
this also all reinforces the mc's childhood trauma of being abandoned and not being able to fully trust his loved ones.
