To me it doesn't make sense, though, because he obviously fought his asthma and had Sebastian train him in all the duties to become the heir. It's obvious that OurCiel steeled himself so he wouldn't give a shit what other people thought, but the whole "I have to be the healthy twin to be seen as a legitimate heir" seems like a bunch of .. grandstanding.
