I think the basis of this "loyalty" thing of the butler is that he was a coward? That even if he knew that ines has mental issue he did not report in immediately to his master i think and that is why ines said that he broke his loyalty.
Secret was Iñez being mentally ill. That's the label to her unknown sickness.
Thw butler knows this but did not disclose it to Carcel which then Iñez labels him not being loyal to his master.
Raul knows. Because a few chapters back they confronted each other (Raul and Butler) and Raul inform Iñez of this which now leads to this conversation in chap 49
You would think there would probably be a start of some kinda character development not just in one character but both after this many chapters in（￣へ￣）

The ending was kinda unexpectedly so touching, i actually teared up a lil HNGGGG, loved the ending although i kinda wish we saw a lil more of the other couple and our main couple but honestly, im totally satisfied with this ;u;
