Bcs his harem it's just a prey that he killed in his mad state. So he didn't consummated them but kill all of them to relieve his bloodthirst i guess.
He seems to be in that state only during the night ? He is shown to be able to finish his work perfectly , he could be finishing someone else too during the day ?

It's already explained in the previous chapter, he got pressured to take concubines to continue bloodline. Bcs of political things He can't refuse. So he made use his mad state excuse to Kill all the concubine that have strong family background that corrupted. Beside he can relieved his bloodthirst, he also can put in check all the nobles, and make their daughter a hostage. He will never lay his hand on the enemy daughter just to give them favourable opportunity agains him but instead kill them one by one using his mad state. He can throw two bird in one stone.
Sorry I'm not good explaining things cuz english is my third language. But that's what i understand abt the story.
