Not sure what happened but to me it seemed like Aunt apologized to him (dogeza,
black clothes, funeral?), so maybe she thinks she's responsible for the parents passing. So Aunt felt extremely guilty having been called "mum", since she took his real mother from him and he was so young that he doesn't even understand that.
However, that is still neglect.
Uncle did poor job though, but exhausted career-driven guy without a partner to lean on could get enough mentally tired while trying to sooth crying child that they end up saying some real hurtful shit.
I feel like the reason for hospital visits that Aunt went to was most likely for mental health/counceling.
